The 14 states
According to the recall notice, the lot was distributed to consumers in Arizona, Colorado, Florida, Illinois, Louisiana, Michigan, New York, Nevada, Oklahoma, Pennsylvania, Texas, Utah, Vermont, and Washington.
Reactions the pharmacy has reported
Optimal Balance Pharmacy says it has received reports of adverse events including fever, chills, severe headache, nausea, vomiting, pain, and symptoms consistent with an allergic reaction.
The recall notice states that there is a reasonable probability that injectable products with elevated endotoxin levels could cause significant medical events including fevers, hypotension, inflammatory reactions, anaphylactic shock, and death.
If you received lot LG342009076
- Stop using it.
- Photograph the vial label: lot number, expiration date, pharmacy name, and your prescription label.
- Talk to a lawyer before you hand the vial back. The pharmacy is arranging to retrieve recalled product and is offering refunds. If you had a reaction, the vial may be evidence, and you should not sign a release tied to a refund before getting advice.
- Get medical care if you are unwell, and tell the doctor what you were given and when.
- Keep the paperwork: shipping box, receipts, and messages with the prescriber or pharmacy.

Part of a larger pattern
Four Texas compounding pharmacies have recalled injectable glutathione since August 2026: Victory Medical Center Pharmacy, Optimal Balance Pharmacy, Centric Compounding, and Greenwich Rx. On August 27, 2026, the FDA said pharmacies tied to adverse event reports had obtained their glutathione from a single lot supplied by Medisca Inc., labeled dietary supplement grade (what the FDA notice says). The CDC has reported more than 25 people sick in at least four states.
